| COMPLETE YOUR APPLICATION Online at https://universalenroll.dhs.gov/ OR call 1-855-347-8371. Keep your confirmation number; you’ll have to provide it when fingerprinted. |
| PAY YOUR FEE The fee is $86.50. You can pay when you apply or at the fingerprinting site. DEBIT/CREDIT CARDS OR MONEY ORDERS PAYABLE TO “MORPHOTRUST USA” ONLY! CASH/PERSONAL CHECKS ARE NOT ACCEPTED. |

If your legal name NOW differs from your Birth Certificate ALSO bring an Original or Certified Copy of a Court Ordered Name Change Document (i.e. Marriage Certificates or Divorce Decree)
